What is a D-SNP?


For individuals enrolled in both Medicare and Medicaid, Dual Special Needs Plans (D-SNPs) offer coordinated care and additional benefits under one plan. These plans offer comprehensive coverage, including benefits not typically available through Original Medicare, all at little to no cost.

Who qualifies for a D-SNP?


You may qualify for a D-SNP if any of the following apply to you:

• Eligibility requires Medicare Part A and/or Part B. You may qualify if you're 65 or older, or under 65 with a serious health condition such as ALS or ESRD.

• You must either be enrolled in your state’s Medicaid program or meet certain income or asset requirements that make you eligible for Medicaid. Some people automatically qualify if they receive Supplemental Security Income (SSI).

Types of Special Needs Plans available

  1. Dual-eligible members are categorized as either Full Dual or Partial Dual. Full Dual members receive the complete range of Medicaid benefits offered by their state, which often includes doctor visits, hospital care, prescriptions, and long-term services. Partial Dual members, on the other hand, may only receive limited benefits, such as help paying for Medicare premiums or certain cost-sharing expenses. Plan availability and enrollment may vary depending on your dual-eligibility status.
