A Chronic Special Needs Plan (C-SNP) is a type of Medicare Advantage (Part C) plan created for people who live with long-term health conditions. These plans are designed to give you the specialized care, drug coverage, and support you need to better manage your health every day.
With a C-SNP from Low-Cost Health Coverage, you get all your Medicare benefits in one plan, plus extra support that’s tailored to your condition, often with $0 costs.
These plans are designed for people living with certain chronic health conditions, like:
• Diabetes.
• Heart disease or heart failure.
• Chronic obstructive pulmonary disease (COPD).
• Kidney disease or end-stage renal disease (ESRD).
• HIV/AIDS.
• Neurologic disorders.
• Autoimmune disorders.
Not sure if your condition qualifies?
We’ll help you check. Our team is here to make sure you get the care and benefits you deserve.
These plans are designed for people who live with ongoing illnesses such as diabetes, COPD, heart disease, or kidney disease. These plans come with coordinated care, custom medication coverage, and useful benefits like dental care, transportation, and more.
If you qualify for both programs, a D-SNP gives you even more benefits at little or no cost. These plans often include prescription coverage, dental, vision, hearing, and extra support to manage your care.
If you live in a skilled nursing facility or an approved assisted living community and require a similar level of care, you may qualify for an Institutional or Institutional-Equivalent Special Needs Plan (IE-SNP). These plans offer close care coordination and access to on-site medical support.

Benefits, features, and medical devices offered may vary depending on your specific plan and where you live. Some benefits may only be available in specific regions. Certain rules and provider network restrictions may also apply.
1. Over-the-Counter (OTC) and Healthy Food Benefits
These benefits may come with expiration dates, so it’s important to use them within the allowed timeframe. The healthy food benefit is only available to members with certain chronic health conditions, such as diabetes, chronic heart failure, or cardiovascular issues. You must meet specific medical and plan eligibility requirements. Please check your plan materials or contact us for more details.
2. Pharmacy Options
You are not required to use a specific mail-order pharmacy to receive medications you take regularly. While we may partner with pharmacies for convenience, you have the option to choose from other participating pharmacies within our network.
3. Insulin Cost Protection
Covered insulin medications cost no more than $25 per month when included in your plan. This cost applies during all stages of your Medicare drug coverage, except during the catastrophic stage, where your insulin may be covered at no cost.
4.Doctor and Provider Networks
The availability of doctors, specialists, and other providers may be different based on your location. Your plan will give you access to a network of healthcare professionals, but it’s important to check whether your preferred providers are included.
5. Rewards Program
Some plans may offer reward programs to encourage healthy actions, like completing wellness visits or screenings. Not all plans include this feature, and rewards may vary. Please check the details of your specific plan for eligibility and program rules.
