Important Disclosures for Prescription Drug Plans
Our prescription plans may include home delivery options for 90-day supplies through partner pharmacies. Home delivery may not be available in certain U.S. territories, such as Guam, American Samoa, the U.S. Virgin Islands, or the Northern Mariana Islands. You are not required to use mail-order services for long-term medications. If it’s your first time using home delivery, you may need to approve your doctor’s order before it can be processed.
Under current guidelines, covered insulin prescriptions will not exceed $35 per month through all payment stages of your plan. Once you reach the Catastrophic Coverage phase, your insulin and other covered medications may have no cost at all.
Plan pharmacy networks include both preferred and standard pharmacies. In rural areas such as parts of Alaska, Montana, Nebraska, North Dakota, South Dakota, and Wyoming, there may be limited access to preferred cost-share pharmacies.
